Job Summary
Ascension Saint Thomas Medical Partners is a robust network of 18 primary care practice facilities in the Middle Tennessee region.
As part of ASTMP’s Addressing SDOH Initiative, we seek a Licensed Counselor who will provide counseling services for enrolled patients who would benefit from this service.
Job Responsibilities
The Addressing SDOH Initiative utilizes a multi-disciplinary team focused on community health needs that will address SDOH barriers limiting regular, preventative healthcare for populations who are avoidant or experience barriers to regular care. This team will include the following:
- Community Health Workers
- RN Care Managers
- Pharmacy Technicians
- Counselors
Qualifications
Bachelor’s Degree leading to licensure in the State of TN to practice as a Licensed Counselor, or Licensed Family Counselor.
Current licensure as a Counselor in TN.
