Job Summary

The Village at Glencliff (TVG) (www.villageatglencliff.org) is hiring a Resident Case Manager who is, first and foremost, passionate about caring for and housing vulnerable populations. We are a relatively new 501(c)(3) organization whose mission is to provide a healing community for medically vulnerable, unhoused Nashvillians where they can safely recover, stabilize their lives, and imagine their futures.

To achieve our mission, we cultivate person-centered services for our residents, who stay with us in one of 12 stand-alone tiny homes from their hospital discharge through recuperation until they are permanently housed. This arc of care seeks to break the cycle of emergency department visits and homelessness. We value equity of care, being curious in problem-solving, and prioritizing sustainability and inclusion in our decision-making.

The role of the Resident Case Manager will leverage the values of The Village at Glencliff with agency and community resources to offer residents every opportunity to break cycles of poverty and unlock pathways to permanent housing, healing, and hope. This is a full-time position, coordinating with other team members to maintain a mission-focused environment in which residents receive the highest quality of compassionate and dignified care while concurrently utilizing trauma-informed care and evidence-based practices to achieve optimum health and housing outcomes in a way that fosters health, mutual respect, clear communication, and positive self-regard.

Job Responsibilities

  • Schedule resident’s medical, mental health, dental, eyecare, and any other necessary appointments; attend appointments with residents or arrange transportation on a case-by-case basis
  • Keep residents informed of scheduled appointments with written and verbal reminders
  • Work with residents to obtain all necessary documents (Photo ID, Birth Certificate, SS Card)
  • Assist residents through the application and/or referral process for available resources (i.e. SNAP, Lifeline phone, bus pass, SSI/SSDI, etc.)
  • Refer residents to all possible housing opportunities, and work with them from start to finish from housing applications to move-in day
  • Refer residents to any other agencies they are eligible to work with for additional support
  • Create and maintain resident data in HMIS and CE for referral purposes; maintain high-quality data for reporting purposes
  • Maintain files, paperwork, and documentation in the office
  • Work with residents (and Program Coordinator) to establish their Care Plan, which addresses their key housing and healthcare goals
  • Coordinate regular meetings to update and revise care plans as goals are accomplished and new goals are realized
  • Ensure that all residents are adequately oriented and accountable to the program rules and guidelines, implementing the Resident Handbook
  • Maintain rules and guidelines onsite; issue conduct warnings and create behavior contracts when rules are broken or behavior issues arise; maintain Grievance Forms; issue Away Passes and Extended Absence form agreements when appropriate
  • Perform daily rounds and house inspections
  • Report maintenance and safety concerns to the Director of Operations
  • Discuss resident concerns with the Program Coordinator
  • Conduct interviews with medically appropriate referrals at hospitals
  • Coordinate paperwork, orientation, and move-in days for new residents
  • Participate in relevant agency and community meetings, events, and continuing education
  • Contribute to TVG newsletters, social media, and other communications as requested
  • Contribute to and manage relevant budget items
  • Other duties as assigned to promote staff and resident success

Qualifications

Required

  • Bachelor’s degree or equivalent experience
  • Commitment to the mission and values of The Village at Glencliff
  • Demonstrated experience working with populations of people experiencing homelessness, transitional housing, housing navigation, addiction, and mental health services
  • Existing connections to local resources including housing, mental health, and addiction services

Essential Skills

  • Excellent organizational, multi-tasking, and time management skills
  • Effective written, verbal, and interpersonal communication skills
  • Demonstrated ability to manage crisis situations compassionately and effectively
  • Cultural sensitivity and adaptability to diverse needs and backgrounds
  • Comfortable in individual, group, and public speaking environments
  • Trauma-informed care, harm reduction, and motivational interviewing
  • Self-motivated with an open and collaborative mindset
  • Broad capacity for creative problem-solving
  • Computer literacy: Office 365 (Microsoft Word, Office, Excel, PowerPoint) mandatory, Social Media fluency considered an asset

Preferred

  • Knowledge of rental housing/real estate, landlord-tenant responsibilities and rights
  • Experience with property management is considered an asset
  • Experience with SSI/SSDI is considered an asset
  • Relevant Undergraduate or Graduate degree, nursing, and/or social work experience an asset
  • First Aid and CPR certifications, or the willingness to become certified

Physical/Mental Demands & Working Conditions

  • Excellent motor vehicle driver with a valid driver’s license and reliable means of transportation
  • Ability to focus and be present in a fast-paced, multi-tasked environment
  • Ability to lift or shift 10-25 pound items as necessary
  • Willingness to be flexible when evening and/or weekend work events arise

Additional Information

Benefits

Salary and Benefits: $43,000 – $46,000, plus monthly reimbursement for health insurance premiums. The Village at Glencliff recognizes 15 holidays per calendar year, and all employees receive three (3) weeks or 15 working days of Personal Time Off annually.
