As an EIS and Intake Specialist at Nashville CARES, you will develop relationships with clients and support them through many stages of life while learning and growing in your own career.

Job Responsibilities

What You’ll Do

  • Represent Nashville CARES in a compassionate and meaningful way as the first contact with individuals seeking services from Nashville CARES
  • Complete full intakes with new clients including biopsychosocial assessment and initial plan of care that supports clients in connecting to necessary services both internally and externally
  • Assist clients newly diagnosed with HIV in seeking out medical care and attending an appointment within 30 days of diagnosis or identification
  • Provide compassionate and respectful care and comprehensive case management services to help clients address the complex challenges associated with living with HIV
  • Meet regularly with clients in the office, in clients’ homes, and in the community
  • Conduct ongoing biopsychosocial assessments on all clients on a semiannual basis
  • Assist clients with connections to medical, social, community, financial, and other needed services
  • Assist clients in developing realistic action steps to achieve their goals
  • Provide support services and referrals to clients who may be socially isolated
  • Advocate for clients within the agency and in external settings, such as medical providers and other agencies they may be engaged within the community
  • Build relationships with medical and social services providers to increase continuity of care
  • Complete all documentation in an appropriate manner and in a reasonable timeframe according to agency standards to ensure clients do not encounter barriers to service
  • Practice appropriate boundaries with clients and with work to ensure you are able to provide excellent services to clients while preserving self-care

  • Nashville CARES is an organization that is passionate about ending the HIV epidemic in Middle Tennessee.
  • We provide prevention services to stop the spread of HIV in the community and medical care and support services to those living with HIV, with the hope of achieving a world where HIV infections are rare, and when they do occur, everyone living with HIV has access to care, treatment, and support to achieve optimal health and self-sufficiency without stigma or discrimination.

  • We provide a vast array of services, including case management, medical care, housing assistance, nutrition and transportation services, and assistance with insurance and prescription needs.

Qualifications

Minimum Requirements:

  • Bachelor’s degree (preference for Bachelor’s in Social Work or other Human Services)
  • Minimum of two years experience in community-based social or health care services – a supervised BSW/MSW internship may count for one year
  • Proficiency with a variety of technology, including Microsoft Office software (Word, Excel, Outlook), electronic health records (training provided), and general Windows PC and cell phone technology
  • Valid Driver’s License, vehicle, and automobile liability insurance to facilitate local travel required for client/agency-related work
